*\*\*Please note that this is a Full-Time position\*\** *Join Elysian Health as a Care Coordinator and Make a Real Difference in Patient Care!* Elysian Health is a growing multidisciplinary healthcare clinic offering Concierge and Executive Health services, along with Allied Health, Imaging, and Diagnostic services. We are seeking a compassionate, organized, and proactive Patient Care Coordinator to join our growing team at our Vancouver location. In this role, you will serve as the primary point of contact for patients, helping coordinate their care journey while ensuring a seamless and personalized healthcare experience. *What You'll Do* * Serve as the main point of contact for patients and provide exceptional customer service * Coordinate appointments, referrals, diagnostic testing, imaging, and follow-up care * Guide patients through their care plans, and ensure next steps are clearly communicated * Liaise with physicians, specialists, allied health providers, and patients to support continuity of care * Maintain accurate electronic medical records and patient documentation * Track outstanding referrals, requisitions, test results, and preventative care requirements * Support clinic operations and contribute to a professional, organized environment *What We're Looking For* * Medical Office Assistant (MOA) Diploma or equivalent experience * Minimum 2 years of experience in a Medical Office Assistant, Patient Care Coordinator, or similar healthcare administrative role * Strong customer service and patient relations skills * Experience working with EMR systems (MYLE experience is considered an asset) * Excellent organizational skills and attention to detail * Strong communication, problem-solving, and teamwork abilities * Ability to prioritize and manage multiple responsibilities in a fast-paced environment * Professional, reliable, and patient-focused approach *Why Join Elysian Health?* * Be part of a collaborative and supportive healthcare team * Work closely with patients in a personalized concierge and executive health environment * Make a meaningful impact on patient care and outcomes * Join a growing organization committed to excellence in healthcare delivery *Please note:* Training may be conducted in South Surrey.
