Position #: 205855 Expected Start Date: To be Determined Union: SUN Facility: Southwest Integrated Health Facility City/Town: Maple Creek Department: Primary Care Sites Type: Full-time temporary Expected Up to Date: October 13, 2027 FTE: 1 Shift Information: Days Number of Hours per Rotation: 14 shifts of 8 hours per 3 weeks Relief: No Float: No Hours of Work: Field Hours Salary or Pay Band: Pay Band Nurse Practitioner $52.590 to $62.910 Travel Required: No Job Description: Reporting to the Manager of Primary Health Care, the Nurse Practitioner provides comprehensive primary health care services focused on health promotion, disease and injury prevention, treatment, and emergency care. Responsibilities include conducting advanced health assessments, developing differential diagnoses, ordering and interpreting diagnostic tests, prescribing and managing pharmacological and therapeutic interventions, and delivering patient-centered care. The role collaborates with clients, families and interdisciplinary health care providers to develop and implement holistic care plans, including prenatal support for at-risk youth. The Nurse Practitioner facilitates a case management approach, supports individuals and communities in addressing health determinants and partners with health care and community organizations to improve outcomes for at-risk populations. The position also contributes to quality improvement initiatives and the enhancement of service delivery models.
